When booking your initial appointment, we do require some additional information. This ensures that nothing is missed or overlooked, seamless continuity with supporting professionals (vets, behaviourists etc.) and consistent regulated treatment is provided towards your beloved animal.

• A completed Vet Referral Form

• An up-to-date Clinical History from you Vet

• A completed Consent to Treat Form

• Insurance Cover Letter
